Universal Chain of Survival

The Universal Chain of Survival describes the key actions that improve survival and recovery after cardiac arrest. These steps apply to people of all ages and emphasize the importance of rapid recognition, immediate care, and coordinated emergency response.

Prevention and Preparedness

Preventing emergencies and being prepared to respond can improve outcomes. Preparation includes recognizing health risks, creating emergency action plans, ensuring access to AEDs, and maintaining CPR and first aid training.

Early Recognition and Emergency Response Activation

Recognize cardiac arrest when a person is unresponsive and not breathing normally or is only gasping. Immediately call 911 or your local emergency number, send someone to get an AED if available, and begin emergency care as quickly as possible.

High-Quality CPR

Start high-quality CPR immediately to help maintain blood flow to the brain and vital organs. Perform chest compressions at the recommended rate and depth, allow complete chest recoil, minimize interruptions, and avoid excessive ventilation.

Rapid Defibrillation

Use an AED as soon as it becomes available. Follow the device prompts, ensure everyone is clear during rhythm analysis and shock delivery, and resume CPR immediately after the shock or when no shock is advised.

Advanced Medical Care, Post–Cardiac Arrest Care, and Recovery

After EMS arrives, trained healthcare professionals will take over care and provide additional treatment. Continue following their instructions and provide any information that may help them care for the person.

Scene Safety and Assessment

Scene safety and assessment are the first steps you take before giving emergency care. By checking the area and the person’s condition right away, you can protect yourself and provide help as quickly as possible.

1. Make Sure the Environment Is Safe for the RescuerBefore helping the person, make sure the area is safe for you and the person. If available, wear personal protective equipment (PPE), such as disposable gloves, to reduce the risk of contact with blood or body fluids—only approach when it is safe to do so.
2. Check Responsiveness, Ask “Are You OK?”Gently tap the person on the shoulders and ask loudly, “Are you OK?” This helps you determine if the person is responsive. If there is no response, act immediately and prepare to provide emergency care.
3. Check for Normal BreathingCheck for responsiveness and normal breathing at the same time for no more than 10 seconds. If the person is unresponsive and not breathing normally or is only gasping, assume cardiac arrest, activate the emergency response system, send someone to get an AED (if available), and begin high-quality CPR immediately.
4. Activate the Emergency Response System (Call 911 or Local Emergency Number).If someone is nearby, ask them to call 911 and bring an automated external defibrillator (AED) while you stay with the person. If you are alone, call 911, put your phone on speaker, and follow the dispatcher’s instructions while beginning CPR as soon as possible.

First Aid Emergencies

First aid is the immediate care given to someone who is injured or suddenly becomes ill until emergency medical services (EMS) arrive. Always make sure the scene is safe, use personal protective equipment (PPE) when available, and provide care within the limits of your training.

Severe Bleeding
Apply direct pressure with a clean dressing. Add additional dressings if needed. For life-threatening bleeding from an arm or leg, apply a commercial tourniquet if direct pressure does not control the bleeding or is not possible.
Shock
Call 911. Help the person lie flat if appropriate, keep them warm, and monitor breathing until EMS arrives.
Burns
Cool thermal burns with cool running water for at least 10 minutes. Cover with a clean, dry, non-stick dressing. Do not apply ice, butter, or ointments.
Bone, Joint, & Muscle Injuries
Keep the injured area still. Apply a wrapped cold pack to reduce pain and swelling. Seek medical care for suspected fractures or severe injuries.
Seizures
If a serious injury is suspected during a seizure, avoid unnecessary movement and wait for EMS. Move the person only if needed to protect their airway or keep them safe from immediate danger.
Severe Allergic Reaction (Anaphylaxis)
Call 911. Help the person use their prescribed epinephrine auto-injector if available. Monitor breathing and begin CPR if necessary.
Asthma Emergency
Help the person use their prescribed quick-relief (rescue) inhaler if available, and you are trained or authorized to assist. Call 911 if symptoms are severe, worsen, or do not improve.
Diabetic Emergency (Low Blood Sugar)
If the person is awake and can swallow safely, give glucose tablets if available. If glucose tablets are unavailable, give another form of fast-acting dietary sugar. Call 911 if the person becomes unconscious, has a seizure, or cannot swallow safely.
Heat-Related Emergencies
Move the person to a cool place. For heat exhaustion, cool the person and encourage fluids if alert. For heat stroke, call 911 immediately and begin rapid cooling.
Cold-Related Emergencies
Move the person to a warm place, remove wet clothing, and cover with dry blankets. Do not rub frostbitten tissue.
Poisoning
If poisoning is suspected, make sure the area is safe before helping the person. Move the person away from the source only if it is safe to do so. Contact your local poison information service or emergency number for instructions. Do not make the person vomit unless directed by a medical professional. Call emergency services immediately if the person has trouble breathing, is unresponsive, has a seizure, or shows signs of a life-threatening emergency.
Bites & Stings
Wash the area, remove a bee stinger by scraping if present, apply a cold pack, and call 911 for signs of a severe allergic reaction.

Adult CPR And AED

Adult CPR and the use of an automated external defibrillator (AED) are critical interventions for a person in cardiac arrest. If the person is unresponsive and not breathing normally or is only gasping, activate the emergency response system, obtain an AED if available, and begin high-quality CPR immediately.

Remember These Key Steps

  • Call 911 and send someone to get an AED.

  • Compress the chest at least 2 inches (5 cm) but not more than 2.4 inches (6 cm).

  • Compress at 100–120 compressions per minute.

  • Allow complete chest recoil after each compression.

  • Minimize interruptions in chest compressions.

  • Avoid excessive ventilation.

  • Continue CPR until an AED is available, EMS takes over, or the person shows signs of life.

Adult CPR: Compressions, Airway, and Breaths

Adult CPR follows three simple steps that help keep blood and oxygen moving through the body during a cardiac arrest. Learning these actions in the correct order helps you respond quickly and give the person the best chance of survival.

C — Compressions

Place the heel of one hand on the center of the chest over the lower half of the breastbone. Put your other hand on top and lock your fingers together. Push the chest down at least 2 inches (5 cm) at a rate of 100 to 120 compressions per minute. Let the chest return to its normal position after each compression, and complete 30 compressions before giving breaths.

A — Airway

After 30 compressions, open the airway using the head tilt and chin lift technique. Place one hand on the forehead and gently tilt the head back while lifting the chin with two fingers. This opens the airway and allows air to reach the lungs more easily.

B — Breaths

Give 2 rescue breaths using a pocket mask or face shield if trained and available. Each breath should be delivered over about 1 second and should make the chest visibly rise. Give only enough air to produce visible chest rise, and avoid excessive ventilation.

AED: Power On, Attach Pads, Deliver a Shock, and Resume CPR

An automated external defibrillator (AED) helps treat certain life-threatening heart rhythms during cardiac arrest. Use an AED as soon as it becomes available and follow the device instructions carefully.

Steps for Using an AED :

1. Power On

Turn on the AED and listen to the voice prompts. The device will guide you through each step.

2. Expose the Chest

Remove or move clothing so the person's chest is bare where the AED pads will be placed. Make sure the area where the pads attach is clear.

3. Attach the Pads

Apply the AED pads exactly as shown on the pad illustrations. For adults, place one pad on the upper right side of the chest and the other pad on the lower left side of the chest.

4. Stand Clear

Make sure no one is touching the person while the AED is analyzing the rhythm or delivering a shock. Clearly say, "Clear," and confirm that everyone has moved away.

5. Deliver the Shock if Advised

If the AED recommends a shock, make sure everyone is clear of the person and deliver the shock according to the device instructions.

6. Resume CPR

Immediately restart CPR after the shock or when the AED advises that no shock is needed. Continue CPR and follow the AED prompts until emergency medical services arrive, the person shows signs of life, or you are unable to continue.

Special AED Situations

  • Wet Chest:
    If the person’s chest is wet, dry the chest before applying AED pads.
  • Excessive Chest Hair:
    If chest hair prevents the AED pads from sticking properly, remove only enough hair to allow the pads to attach.
  • Infants and Small Children:
    Use pediatric AED pads when available and follow the AED pad placement instructions. If AED pads may touch because of the person’s size, place one pad on the chest and the other pad on the back.

CPR Ratio: 30 Compressions to 2 Breaths

During adult CPR, follow a cycle of 30 chest compressions followed by 2 rescue breaths. Repeat this 30 to 2 pattern without stopping until an AED is available, emergency medical services take over, or the person begins to respond.

Early CPR and Early AED Mean Better Survival

When someone has a sudden cardiac arrest, time is critical. Starting CPR right away and using an AED as soon as possible can double or even triple the person’s chance of survival because these actions help keep blood flowing and restore a normal heart rhythm.

Child and Infant CPR

Children and infants can experience cardiac arrest for different reasons than adults, but the basic goal of CPR remains the same: provide chest compressions and breaths to help move blood and oxygen through the body.

The main CPR differences are based on the size of the person and the rescuer’s ability to provide effective compressions.

Person Key CPR Differences
Adult Use two hands for chest compressions in the center of the chest.
Child (1 year to puberty) Use one or two hands depending on the child’s size and the rescuer’s ability to provide effective compressions.
Infant (Birth to 1 year) Use two fingers for single-rescuer CPR or the two-thumb encircling technique when two trained rescuers are available.

Child and Infant CPR Steps

  1. Check Responsiveness and Breathing
    • Check if the child or infant responds and is breathing normally.
    • If they are unresponsive and not breathing normally or are only gasping, begin CPR.
  2. Call for Emergency Help
    • Activate the emergency response system and get an AED if available.
    • If you have a mobile phone, call emergency services on speaker while beginning CPR.
  3. Give Chest Compressions
    • Provide compressions at a rate of 100–120 per minute.
    • Compress the chest about one-third of its depth.
    • Allow the chest to return fully after each compression.
  4. Give Rescue Breaths
    • Give 2 breaths after compressions.
    • Each breath should make the chest visibly rise.
    • Avoid giving excessive breaths.
  5. Use an AED
    • Use an AED as soon as it is available.
    • Follow the AED instructions and use pediatric pads when available.

Continue CPR until:

  • The AED is ready and instructs you to stop,
  • Emergency medical services take over,
  • The child or infant shows signs of life, or
  • You are unable to continue.

Rescue Breaths During Child CPR: Open, Breathe, Watch, and Repeat

Giving breaths during child CPR helps provide oxygen to the body when a child is not breathing normally. Follow the steps carefully and continue CPR with a 30 compressions to 2 breaths cycle until an AED is available or emergency medical services arrive.

1. Open Airway
Open the airway using the head tilt–chin lift technique. If you suspect a head, neck, or spine injury, use care when moving the person’s head, but do not delay opening the airway if rescue breaths are needed.
2. Give Breaths
Give 2 rescue breaths using a pocket mask or face shield if trained and available. Make sure each breath creates a gentle rise of the chest.
3. Watch the Chest
Watch the chest as you give each breath to confirm that it rises. Provide only enough air to make the chest visibly rise, and avoid giving too much air.
4. Repeat the Cycle
Continue CPR using the correct compression-to-breath ratio:
  • Single rescuer: 30 compressions followed by 2 breaths.
  • Two trained rescuers: 15 compressions followed by 2 breaths.
    Continue CPR until an AED is ready for use, EMS takes over, or the child begins to respond.

Choking Relief (Conscious & Unresponsive)

Choking can block the airway and prevent a person from breathing properly. Learning the right steps for conscious and unresponsive choking helps you respond quickly and provide effective help during an emergency.

An opioid-associated emergency can cause a person to become unresponsive and stop breathing normally. Quick action can help save a life.

Recognize the Emergency

An opioid emergency may be suspected if a person is: 

  • Unresponsive or difficult to wake.
  • Not breathing normally or only gasping.

Respond to an Opioid Emergency

  • Call emergency services immediately.
  • Get an AED if available.
  • Give naloxone if it is available and you are trained, authorized, or permitted to use it.
  • Begin CPR if the person is unresponsive and not breathing normally.
  • Continue care until emergency medical services arrive or the person shows signs of life.
